Participant Waiver and Release of Liability
Assumption of Risk
I acknowledge that participation in activities at Full Steam Ahead Play and Learn Café involves inherent risks, including but not limited to physical activity, interaction with play equipment, and participation in group events. I voluntarily assume all risks associated with these activities on behalf of myself and any minor(s) listed below.
Release of Liability
In consideration for being permitted to enter and use the facilities at Full Steam Ahead Play and Learn Café, I, on behalf of myself and any minor(s) listed below, hereby release, waive, and discharge Full Steam Ahead Play and Learn Café, its owners, employees, agents, and representatives from any and all claims, liabilities, or demands arising out of or related to any loss, damage, or injury, including death, that may be sustained while participating in activities or using the facilities.
Supervision
I understand that I am responsible for supervising my child(ren) at all times while on the premises of Full Steam Ahead Play and Learn Café. I agree to ensure that my child(ren) adhere to all posted rules and guidelines.
Health and Safety
I certify that my child(ren) are in good health and free from any contagious diseases. I agree not to bring my child(ren) to the facility if they are exhibiting signs of illness.
Emergency Medical Treatment
In the event of an emergency, I authorize Full Steam Ahead Play and Learn Café to seek medical treatment for my child(ren) and agree to be responsible for any costs incurred.
Photo and Video Release
I grant Full Steam Ahead Play and Learn Café the right to take photographs and videos of me and my child(ren) for promotional purposes, without compensation.
Electronic Signature
By signing below, I acknowledge that I have read and understood this waiver and release of liability, and I agree to its terms. I understand that my electronic signature is legally binding.
